What is Chapter 05 of ICD-11?

The Chapter 05 group the endocrine, nutritional or metabolic diseases. It is one of the chapters with the greatest global clinical relevance given that it includes diabetes mellitus, thyroid disorders, obesity and metabolic disorders — conditions of very high prevalence throughout the world.

In the ICD-10, this content corresponded to Chapter IV (E00–E90). ICD-11 reorganizes it into four main sections:

Key change vs ICD-10: Diabetes mellitus was significantly expanded to reflect current international terminology (WHO classification 2011). Categories were added for intermediate hyperglycemia (prediabetes) and insulin resistance syndromes. Diabetic complications follow in the corresponding body system chapters.
Important: The neoplasms of the endocrine system They are found in Chapter 02 (Neoplasms), not here. The symptoms and clinical findings of endocrine diseases are in Chapter 21.

Section 1 — Endocrine Diseases

Organized by specific gland or hormonal system. Includes thyroid, pancreas, pituitary, adrenals, parathyroid and gonads.

5A00–5A0Z
Disorders of the thyroid gland or thyroid hormone system
›

Includes hypothyroidism, thyrotoxicosis (hyperthyroidism), thyroiditis and other thyroid disorders. The structure did not change from ICD-10, but the terminology was revised to better reflect current disease processes.

5A0ZDisorders of the thyroid gland or thyroid hormones system, unspecified
5A10–5A2Z
Diabetes mellitus and intermediate hyperglycemia
›

The most consulted block of the chapter. The ICD-11 expands the types of diabetes and adds intermediate hyperglycemia (prediabetes) as its own entity. Diabetic complications are coded in the affected system chapter, linked to the type of diabetes using the "Code also" instruction.

CodeDiagnosis

5A22Diabetic acidosis
5A23Diabetic coma
5A41Hypoglycaemia without associated diabetes
WHO Coding Instruction: Type 2 diabetes (5A11) does not accept other causes, except conditions that cause insulin resistance. Type 1 diabetes (5A10) only accepts conditions that cause autoimmune destruction of beta cells. Diabetic complications (retinopathy, nephropathy, neuropathy) are coded in the affected system chapter using “Code also.”
Post-coordination diabetes + complication: When a patient with type 1 diabetes has diabetic acidosis, the cluster order is: 5A10/5A22 (etiological cause first, complication later).

ICD-11 disaggregates the E20–E35 block of ICD-10 into more specific sections: parathyroid, pituitary, adrenal, gonads, puberty, and polyglandular. Each section follows the affected gland or hormonal system.

5B71Protein deficiency
5B7ZUnspecified undernutrition
5B80.01–5B8Z
Overweight, obesity or excesses of specific nutrients
›

One of the most frequently coded blocks in primary care and internal medicine. The ICD-11 offers greater specificity for types of obesity in adults, children and adolescents.

CodeDiagnosis
5B80.01Overweight in adults
5B81.01Obesity in adults
5B81.0Obesity due to energy imbalance
5B81.1Drug-induced obesity

Section 3 — Metabolic Disorders

ICD-11 classifies metabolic disorders by etiology, in three broad areas, unlike the ICD-10 which used a less systematic organization.

5C80–5C8Z
Disorders of lipoprotein metabolism and certain lipidemias
›

Dyslipidemia and cholesterol and triglyceride disorders. One of the most codified blocks in cardiology and preventive medicine.

CodeDiagnosis
5C80Hyperlipoproteinaemia
5C80.00Primary hypercholesterolaemia
5C80.1Hypertriglyceridaemia

5C8ZUnspecified disorders of lipoprotein metabolism or lipidaemias
5C50–5C7Z
Inborn errors of metabolism and acquired metabolic disorders

Frequently asked questions

What is the ICD-11 code for type 2 diabetes?
Diabetes mellitus type 2 is 5A11 in ICD-11. In the ICD-10 it corresponded to E11. Type 2 diabetes does not accept other conditions as the basic cause of death, except those that cause insulin resistance.
What is the ICD-11 code for type 1 diabetes?
Type 1 diabetes mellitus is 5A10 in ICD-11. In ICD-10 it was E10. It only accepts as a cause conditions that produce autoimmune destruction of the beta cells of the pancreas.
What is intermediate hyperglycemia in ICD-11 and what is its code?
Intermediate hyperglycemia (prediabetes) is a new entity in ICD-11, with code 5A40.Z. It did not exist as an independent category in the ICD-10. It includes impaired fasting glucose regulation and glucose intolerance.
How are diabetes complications coded in ICD-11?
Diabetic complications (retinopathy, nephropathy, neuropathy, diabetic foot, etc.) are coded in the chapter of body system affected, not in Chapter 05. The "Code also" instruction links the complication to the type of diabetes. For example, diabetic cataract in Chapter 09 + 5A10 or 5A11 clustered.
What ICD-11 code does hypothyroidism have?
5A00 is the hypothyroidism category. Its subcategories distinguish congenital, acquired and other forms. The exact code depends on the documented cause and clinical details.
What is the ICD-11 code for obesity?
5B81 is the obesity category. 5B81.01 is obesity in adults. Select the appropriate category and any required extensions using the documented clinical details; the subcodes 5B81.0 and 5B81.1 are not BMI grades I and II.
What ICD-11 code does gout have?
Gout is FA25, in the chapter on diseases of the musculoskeletal system or connective tissue. More specific codes depend on the form of gout and the affected site.
What ICD-11 code does Cushing's syndrome have?
Cushing syndrome is 5A70. Use the official coding tool to select a more specific subcategory when the cause is documented.
